Upon scheduling a $1 donation payment using Internet or Mobile Bill Pay, to one of four charitable causes pre-defined by U.S. Bank, a customer will receive a $1 credit to their checking account within 3-4 weeks following processing of the donation payment. The donation payment must be (1) the first Internet Bill Pay payment scheduled; (2) no more than $1; and (3) processed and verified for credit to apply. Eligible customers may qualify for an additional $20 credit by paying three (3) bills using Internet Bill Pay within 60 calendar days following the scheduling of the donation payment. All payments must be processed, not scheduled or pending, to qualify. Payments less than $10 or payments made back to the owner of the eligible checking account do not qualify. Payments must be verified for credit to apply, which could take 3-4 weeks following processing of all three additional payments. Account must be open and in good standing to qualify for promotional credits. Credits will appear as “Bill Pay Giving Program Reward” in the customer’s transaction history and will be reported as miscellaneous income on IRS Form 1099-MISC if the annual miscellaneous income from U.S. Bank exceeds the IRS reporting limit. Account holder is responsible for any applicable taxes. Only Bill Pay transactions initiated through U.S. Bank Internet Bill Pay or Mobile Bill Pay are eligible. Bill payments made by automated phone banking or Professional Management Software are not eligible. Bills paid using Single Point Bill Pay are not eligible. Each customer can receive the donation payment reward and additional payment reward only once. This is a limited time offer. The donation payment must be scheduled on or before June 30, 2012 in order to qualify for the cash incentive. Donations from the Bill Pay Giving Program in all marketing channels will be matched by U.S. Bank up to a total of $50,000 in 2012.
