To qualify, customers must enroll in U.S. Bank Internet Banking and enroll their consumer checking account in online statements between November 1, 2011 and November 30, 2011 (the "Promotion Period"). $10 will be deposited directly into the qualifying customer's checking account on or before December 15, 2011. $10 will be reported on IRS form 1099-MISC and the customer is responsible for any applicable taxes.
