In responding to this offer, you are requesting a David's Bridal credit card from Household Bank (SB), N.A. (hereinafter "Household Bank", "we", "our", or "us").
You agree that Household Bank or our affiliates may check credit information on you from whatever source we choose, including obtaining a current credit bureau on you in connection with our review of your Acceptance Certificate. You further understand that by signing
this Acceptance Certificate, you agree that purchases on your Account will be subject to the terms and conditions in the Cardholder Agreement (which provides an arbitration provision). You acknowledge that you have read and received a copy of the Cardholder Agreement and Disclosure before making any
purchases under this account. You must be at least the age of majority to be eligible and the issuance of a credit card is subject to verification of all information you have supplied. Your current mailing address must match the address printed on the Acceptance Certificate. Two forms of ID are required to process your application. The first form of ID must be a driver's license, State ID
or active Military ID. The second form of ID must be a major credit card or US Passport. You received this offer because you satisfied our criteria of creditworthiness. If you accept this offer, we may refuse to extend credit if you no longer meet these criteria or applicable criteria bearing on your creditworthiness. This offer is void if you currently have a David's Bridal credit card. You must respond by the expiration date set forth on the Acceptance Certificate. Any response or pre-approved*Acceptance Certificate received after the expiration date will be processed as a non-preapproved application.
Information contained in your credit report obtained from a credit reporting agency was used in connection with selecting you for this offer.
